   • Identify and thoroughly understand the barriers affecting their ability to access, use, and sustain their supports.

   • Develop and implement solutions to address these barriers effectively.

   • Prioritise the most critical factors that need immediate attention and identify the necessary support systems to overcome these barriers.

Customised Service Plans: For participants with complex support needs, we design personalised service plans that ensure all services can work together harmoniously to help the participant achieve their goals. We establish clear processes that facilitate communication, cooperation, and collaboration among both funded and non-funded supports within the participant's network.
